RFI - Hawkes Bay District Health Board (NZ) - Referrals Management

Hawke’s Bay DHB is looking for ways to deliver better health outcomes for its population by delivering integrated and coordinated services within the district and across the central region and ultimately nationally.

Hawke’s Bay DHB requires processes and information systems that enable collaboration between providers of health services, within and across Hawke’s Bay DHB. Improving the population’s health requires a “whole of system” approach, requiring coordination and collaboration across multiple providers, clinical disciplines and service delivery channels.

HBDHB wish to create a core set of personal health information available electronically to patients and their treatment providers, regardless of the provider setting as they access health services.

The key principles in delivering this vision are:

• Single view of the patient’s electronic health record
• Information available to all health care providers
• Information gathered as part of the process
• Electronic communication
• Standardized care pathways
• Enables workflow
• Business intelligence, clinical audit and reporting capability
